**For non-traditional students who do not live on campus or communally, the MCV vaccine is optional, but you must note an exemption on the form in the appropriate space.

If you are having difficulty finding your vaccination records, the CDC recommends the following:

1. Check with your high school for dates of any immunizations. Keep in mind that generally records are kept only for 1 to 2 years after students leave the school.
2. Check with previous employers (including the military) that may have required immunizations.
3. Check with your doctor or public health clinic. Keep in mind that vaccination records are maintained at a doctor’s office for a limited number of years.
4. Contact your state’s health department. Some states have registries (Immunization Information Systems) that include adult vaccines.

Yes! You should be able to access your previously submitted records in Etrieve to assist you in the completion of a new Health Immunization Form. Please carefully review the requirements to ensure you are compliant and up to date.

Please DO NOT send your immunization record to the Health Center. You must upload it into Etrieve for it to be reviewed.

Athletes: Check your ARMS profile to obtain vaccination records so that they are up to date.

If you aren’t uploading your official vaccination record from your physician or your public health clinic, you need to make sure your primary care provider has signed your Health Immunization Form attesting that the vaccinations and dates you provide on the form are accurate as listed.

Please double check that you are submitting vaccination records for all required vaccines that match the dates you provided on the form. Insufficient vaccinations or incomplete records are the main reason a form is not approved on the first attempt.

No. If you are in the process of completing required vaccinations, please wait to upload your form into Etrieve until the vaccination process has been completed. We cannot approve future vaccine dates.

If you would like to request exemption from a vaccination, there is a place to indicate this on page 2 of the Health Immunization Form.

1. You will need to write the word “Exempt” next to the individual vaccine/vaccines you are claiming exemption from. You need to sign and date the bottom of the form under Statement of Exemption or your form will be rejected.
2. By requesting exemption, you acknowledge that in the event of an outbreak of vaccine-preventable communicable disease, you may be asked to quarantine off college campus for safety and transmission mitigation efforts.
3. If you decide to receive vaccinations that you previously requested exemption from, please upload proof of vaccination to Etrieve so we may update your status.
